    TheAmericanRecoveryandReinvestment(ARRA)Actof2009introducedsweepingreformsin

    thehealthcaresector.Oneofitsmajorprovisionswasthecreationoffinancialincentivesfor

    migrationfrompaperorhomegrownsystemstointegrated,standardsbasedElectronicHealth

    Record(EHR)systemsby2014.Infact,a2010surveyshowedthatEHRdeploymentstoppedthe

    listofprioritiesforhealthcareITteams.Thistrendisusheringthehealthcareindustryintothe

    bigdataerawithmanybenefitsintheareasofmedicaldataexchange,diagnoseserror

    reduction,andoverallpatientcarequality.Digitizeddata,however,isaccompaniedbyclear

    challengesandobstaclesthatmustbeaddressedinordertobenefitfromthenextgenerationofhealthcaretechnology.

    Data Management Inefficiencies

    TheadoptionofcommercialstandardsbasedEHRapplicationsintroducessignificanthardware,

    software,storage,anddatabasecosts.Thesheervolumeofdataalsonecessitatesmore

    manpowertomanageapplicationsandunderlyingdatastores.Inthiscontext,organizations

    needtoconsidernotonlytheproductiondatabasebutalsothenumerouscopiesrequiredfor

    applicationdevelopment,QA,support,training,reportingandotherpurposes.InanEnterpriseStrategyGroupsurvey,over50%ofrespondentsindicatedtheycreateupto10copiesoftheir

    productiondatabaseforsecondaryusage.

    Inhealthcare,lengthydataretentionperiodsandconsumerrightstorequestcomprehensive

    accessreports,asmandatedbytheHIPAAAct,leadtodatabeingstoredinonline,analysis-

    friendlydatabases.Associateddatabasecostsarecompoundedforhealthcareprovidersbecause

    dataschemasandformatsvaryfromonedepartmenttoanother(e.g.radiologyvs.oncology).It

    isnotuncommonforalargehospitaltousedozensofdifferentclinicalapplicationvendors.On

    thebackend,theseapplicationsmayrequiresupportingdifferentdatabasesandevenspecific

    versions.

    Themultiplyingeffectofaheterogeneousapplicationenvironment,longretentionperiods,and

    numerousproductioncopiesmakesdatabaseoverheadcostsabarriertoEHRadoption.Largely

    anon-profitindustry,thehealthcareprovidersectorcannotassumeitsITbudgetswillgrowat

    therateofdigitaldata.Realizingthebenefitsofelectronicmedicalrecordswillrequire

    technologythatallowshealthcaredatatoscalewithoutproportionalincreasesindata

    managementcosts.

    Application Lifecycle Paralysis

    JustassoonashealthcareprovidersbeartheinfrastructurecapitalcostsfornewEHRsystems,

    theywillhavetofacetheunavoidableoperationalcostsandprocessinefficienciesthat

    accompanybigdatatransformations.Beyondtheinitialrollout,providerswillneedtocontend

    withincreasingdataanalysisrequestsforproactivediagnosticdatamining,hospitalefficiency

    reporting,andclinicalstudies.Similarly,enduserswillpushforaccesstofunctionalityavailableinnewversionsofclinical,billing,andotherapplications.ForITdatamanagementteams,this

    translatesintomoredatabasecopiesandfrequentrefreshesfordevelopment,testing,staging,

    assurance,support,reporting,training,etc.Yet,today,theaverageenterprisecantakeseveral

    weeksjusttoprovisionasingledatabasecopyintoanewenvironment.

    ManyhealthcareorganizationsarealsoconsideringSaaSEHRapplicationsoffereddirectlyby

    clinicalapplicationvendorsortheirpartners.Whilecloudbasedofferingsenjoygreater

    efficienciesindatamanagementfromeconomiesofscale,theservicelevelstheyguaranteeare

    inherentlylimitedbythesamedatabasemanagementchallenges.Totrulydeliverdataasa

    serviceandmeetenduserexpectations,healthcareITteamsneedfarmoreagilityregardlessof

    whetherthedataisstoredintraditionaldatacentersorpushedtothecloud.

    Uncontrolled Proliferation of Sensitive Data

    Thehugemarketforstolenidentitieshasmadehealthcareorganizationsatargetof
