Overview
• An initiative to improve oral health and healthcare for older persons within the counties of Kent, Surrey and Sussex.
• Interdisciplinary and multi-sectorial involvement of many stakeholders and third sector parties
• HEKSS led training and education package plus parallel project assessment and evaluation in association with the University of Kent
• Beginning with care home residents and assuring outcomes using oral health care tools:

* A risk assessment to identify problems and risk factors
* Individual assessment of need
* Facilitates appropriate response
* Reflects good practice
* Based on guidance
* Contributes to planning and delivery of daily care
When should it be done?
*On admission, as a matter of routine OR
*As soon as this practice is adopted AND
*At specified intervals to monitor changing needs
Who should do it?
*Any member of the team

*Self assessment by able residents
