１ Public medical insurance (Koteki iryou hoken)

Public medical insurance is a mutual-aid system to reduce the burden of medical bills on the sick or injured through cost sharing, and

everyone living in Japan is obliged to enroll.

There are two categories of public medical insurance programs: Employee Health Insurance, in which company employees are enrolled,

and National Health Insurance and Medical Care System for People Aged 75 and over, which applies to those not eligible for Employee

Health Insurance. Under these systems, the insured party pays only 30% or less of medical expenses at the hospital where treatment for

illness/injury is sought.

① Social Insurance (shakai hoken), Mutual aid (kyosai), National Health Insurance Society (kokumin kenko hoken kumiai) (for

those aged 74 and younger)

Those employed by companies are enrolled in this health insurance program. The company handles the application procedure and

automatically deducts premiums from salaries.

Showing an insurance card at the hospital allows the bearer to pay just 10 to 30% of the medical expenses incurred.

② National Health Insurance (kokumin kenko hoken) (for those aged 74 and younger)

Persons eligible for residence registration in Sapporo (see page 26) who are not enrolled in any form of workplace insurance are

obliged to enroll in the National Health Insurance Program. (If you entered Japan with a designated activities visa for situations such

as receiving medical care, accompanying someone receiving medical care, tourism/vacation or temporary work transfers, you may be

exempted from applying for national health insurance if your country is part of the Social Security Agreement. Please visit your ward

(page 96) for further details.)

The relevant procedures can be completed at ward offices, and require the following:

・A form of identification such as your residence card and passport

・If you plan to have your insurance premiums paid by bank transfer, bring your bankbook, registered seal, and/or cash card (this

procedure could be completed at later dates for those that do not have a bank account at the time).

If your address changes or you have a new baby, you must complete the related procedures to update your information at the ward

office within 14 days.

Insurance premiums

Premiums are calculated based on the number of family members and the previous year’s income in Japan of the person insured.

Payment of premiums for the year is made in 10 installments. As a general rule, this is paid with automatic remittance using the

account transfer systems provided by financial institutions.

Insurance cards

One health insurance card is issued per individual. Showing the card at the hospital allows the bearer to pay just 10 to 30% of the medical

expenses incurred. However, medical examinations, preventive shots, cosmetic surgery, and other cases not defined as medical treatment of

illness/injury are not covered by this insurance, and must be paid in full.

Health insurance benefits

Benefits are provided if the insured party gives birth, dies or incurs large medical bills due to hospitalization or for other reasons.

③. Medical Care System (koki koureisha iryou seido) (for those Aged 75 and over)

Those aged 75 years and older who are eligible for residence registration in Sapporo (see p. 26) are obliged to enroll in the Medical

Care System (If you are in Japan temporarily with a designated activities visa for situations such as receiving medical care,

accompanying someone receiving medical care, or temporary work transfers, you may be exempted from applying for national health

insurance if your country is part of the Social Security Agreement. Please visit your ward or your ward’s website (p.96) for further

details.)

Each person is issued 1 insurance card. When the card is presented at a medical facility, treatment for illness can be received with a

self-pay ratio of 10%. Those whose income is above a certain level, however, pay 30%.

No procedures are required, except in cases where those between 65 and 74 years of age with certain disabilities are enrolled. The

amount of premiums will depend on the receiver’s income.

２ Reduction of medical expenses

If any of the following conditions apply, enrollment in public health insurance allows reduced payment of medical expenses

(income-tested). Applications for the reduction must be made at the relevant ward office (p. 96) beforehand.

●People with a high degree of mental/physical disability

●Single-parent families with children under 20 years of age

●Children that haven’t graduated junior high school (p. 56)

３ Long-term care insurance system (Kaigo hoken)

Under the long-term care insurance system, people aged 40 or older are obliged to enroll and pay insurance premiums. Should they need it,

they are eligible to use services such as care at home or in institutions, rent disability-assisting equipment and to receive placement in welfare

facilities for the elderly needing care, paying only 10 or 20% of the medical costs charged for such services.

Those aged 40 years and older who are eligible for residence registration in Sapporo (see p. 26) are obliged to enroll in the long-term

care insurance program. Premiums are based on income.

The care services covered by the insurance vary by age and health condition. The use of these services requires application at the relevant

ward office (p. 96) and certification as being in need of such care.

４ If you become ill

Hospitals/clinics are generally open only during daytime hours on weekdays, and are closed on Saturday afternoons, Sundays and

national holidays. Some hospitals operate during nighttime hours and on weekends.

Waiting times may be long in crowded hospitals.

Receiving medical care at a hospital

② After examination and treatment, pay your bill at the cashier and collect any prescription issued.

③ Take the prescription to a pharmacy to buy medicine.

６ Sapporo City General Hospital

Sapporo City General Hospital (p. 96) is operated by the City of Sapporo. Basic reception hours for patients are from 8:45 to 11:00

and from 13:00 to 15:00 Monday through Friday, although these hours do not apply to some departments on particular days. Because

some medical departments conduct outpatient care on preliminary referral, please contact the hospital directly for details. If you need

a volunteer interpreter, please consult the hospital in advance.

７ Welfare assistance

While foreign nationals are not eligible to receive welfare assistance, in cases where permanent residents, the spouses of Japanese

citizens/permanent residents or long-term residents are struggling with their livelihood and are unable to receive the necessary

support from their country’s Consulate General, etc., it is possible to receive assurance of livlihood based on standards and conditions

outlined within the welfare assistance program. For those in need, please contact the ward office (p. 96) that has jurisdiction on the

address that is found on your Residence Card or Special Permanent Resident Certificate.

８ Pension system

Under the pension system, the insured party pays premiums to receive benefits after retirement and in the event of becoming unable

to work due to disability or death, in order to support the livelihood of the insured and their family.

① National Pension Plan (Kokumin Nenkin)

All residents of Japan aged between 20 and 59 are obliged to join this basic pension plan. Procedures are completed at the ward

office (p. 96) (except for residents from countries exempted by the relevant treaty).

Premiums can be paid automatically using the account transfer systems provided by financial institutions, or directly at financial

institutions or convenience stores, by credit card, or by electronic payment systems.

For company employees, government employees and teachers (including their dependent spouse), the employer performs the

necessary procedures; such workers do not need to pay premiums individually.

② Employees’ Pension/Mutual Aid Pension Programs (Kosei Nenkin)

In addition to the National Pension Plan that everyone is obliged to join, the Employee’s Pension Program is also compulsory for

those employed by companies, and public servants and teachers. The employer performs the necessary procedures, and premiums

are withheld automatically from monthly salaries.

③ Receiving pensions

Pensions are paid to those aged 65 years or older, with disabilities, and families of insured parties who have died. Payments are

dependent on the fulfillment of fixed criteria including the length of time for which premiums have been paid.

④ Lump-sum withdrawal payments (dattai ichijikin)

Foreign nationals who belonged to the National Pension Plan/Employee’s Pension Program and paid premiums for six months or

more can receive a lump-sum withdrawal payment provided that it is claimed within two years after leaving Japan. For details, contact

the relevant social insurance office.

⑤ Social Security Agreement

People living in Japan that are 20-59 years of age are obligated to be under Japanese pension programs. However, if you are already

registered under your home country’s social security system, you may run into issues having to cover for insurances for both countries

or have to pay for premiums which offer no refund payments if you stay for a short period of time. In order to prevent such problems,

this Social Security Agreement has been organized.

Goals of the Social Security Agreement:

①To adjust pension systems among countries to prevent a double burden of pension fees

②To enable receiving pension benefits from home countries and prevent not being able to receive insurance

refunds by transferring the number of years enrolled in Japanese pension systems as years contributed to the pension systems of

the home country (transferring of pension program participation years).

The countries participating in the Social Security Agreement as of July 2016 are listed below. Japan has made this agreement with 19

countries. Please be aware that “double burden prevention” and “the transferring of pension program participation years” are only

agreed by the following countries.
