What is the Intent to Apply Process?

The intent to apply is your declaration to the Health Professions Office (HPO) and the Health Professions Committee (HPC) that you intend to apply to health professional schools for this upcoming application cycle.

The application is 23 pages in length. Much of the information that you must provide will also be useful as you prepare your professional school application.

The information in the intent to apply application assists the committee in composing a letter on your behalf.

Letter Packet vs. Committee Packet

A letter packet does not include a committee letter of evaluation.

completed.

The full ItoA process will be largely directed towards applicants who are requesting a committee packet. A committee packet contains a committee letter and all other letters of evaluation requested.

Committee Letter Eligibility

Full-time or part-time matriculated CSUF students, alumni, and non-matriculated students who have completed the majority of prerequisite coursework at CSUF.

Submitted all of the required documents by 5 PM on Monday, January 13, 2014.

Résumé or CV

Unofficial transcripts from all institutions attended. If courses from one institution are also listed on the transcript of another institution, there is no need to submit both transcripts. Please note, these transcripts are for internal purposes only and cannot be forwarded to the centralized application service or the schools themselves.

At least (1) letter of evaluation from a health professional. Please note that a letter documenting a minimum of 50 hours in a CCE program or

similar program can substitute for a health professional’s letter of evaluation

Digital photograph (JPG file – low resolution).

Fee payment

File Fee

$20 per professional program for current students and recent alumni (graduated less than one year).

$100 fee to apply to multiple professional programs for alumni who graduated more than one ago.

File fees can only be paid through the Health Professions Advising website or at the Student Financial Services (SFS) Window (UH-180).

File fees are non-refundable and cannot be rolled over to future ItoA application cycles.

TITANium helps you digitally track your file’s progress. Students are automatically added to the HPAO’s TITANium site in 1-3 business days upon submission of the ItoA application.

Please pay special attention to the notes that the HPO leaves for you on TITANium.

All necessary documents can be downloaded/uploaded from TITANium. A quick tutorial on how to upload a document can be found on our YouTube channel.

Please check your preferred email on a daily basis, as this is our main way of communicating with our students.

Intent to Apply Application

Please complete the entire application. If any area is left blank, the application will be returned.

The application is to help prepare you for your submissions to AMCAS, AACOMAS, AADSAS, etc. and as such should be completed with the same amount of care.

At least one letter from a health professional is due before 5 P.M. on 1/13/2014.

The committee review will never be a factor in delaying your application.

A chance to have a critical review of your application by faculty before submitting it to professional school.

The HPO will follow up with letter writers if something is missing or incorrect.

Future Applicants

Applications are accepted year-round for those who only wish to have the HPO collect letters of recommendation.

Only required to complete pages 3-6 of the ItoA application (no fee required).

The Intent to Apply file will close each November.

Resubmit the application every year to keep your file active and updated.

Rollover

Applicants who do not receive admission or do not complete the application process will be

automatically rolled over to the next ItoA cycle upon receipt of the Intent to Apply Re-Applicant

Form and all necessary documents.

Missed Deadline

If you fail to submit or request all of the required items by 5 P.M. on January 13, 2014, you will not receive a committee letter of evaluation for the upcoming application cycle.

The office will however still upload your letters in the form of a letter packet to the professional schools to which you are applying.

What’s New

The Reference Request Form is now no longer required with each letter of evaluation submitted. The form now serves as a helpful guide thus it should still be passed along to all your letter writers.

The autobiography portion of the ItoA has been removed and is no longer required

Now in addition to institutional course GPA information we now require all pre-requisite course grades

What’s New (Cont.)

Now a self assessment table has been added to the application asking students to rate themselves in 8 different areas (ex. Critical thinking, professionalism, etc.)

An ethnicity and race identification page has now been added to the application

Re-Applicants now have a different submission date of May 5, 2014 in which they are required to submit the Re-applicant Form and all other required materials.

University of Minnesota Workshops

It is highly encouraged for students to review the University of Minnesota Workshops before they submit their ItoA application.

The workshops touch upon personal statements and planning for professional school
