SAF,T will work in partnership with DCF to foster the development of a culturally competent and responsive service system that promotes the safety and wellbeing of Aboriginal children, youth and families. To achieve this, SAF,T will auspice the establishment and operation of the Aboriginal Child Youth &

Family Services (ACYFS) in Darwin and Alice Springs.

SAF,T will ensure that its staff and contracted providers have a valid Working with Children Clearance as per the Care and Protection of Children Act.

SAF,T will have in place an internal complaints mechanism and occupational health and safety policy.

SAF,T will have in place recruitment policies and procedures that ensure the safety and security of all clients and a system of criminal history checks for all

staff working with vulnerable people.

SAF,T will have in place an appropriate Asset Management Policy and will maintain an asset register as per clause 12 of the Agreement (01/07/2012 to

30/06/2015).

SAF,T understands the need to ensure the funded service properly represents its client base and promotes diversity in staffing, management and organisational governance. During the term of this Service Agreement, SAF,T agrees to implement strategies to actively recruit and retain staff and board

members from Aboriginal and Culturally & Linguistically Diverse background into Service Delivery and Senior Management positions and/or into Office Bearer positions, in a way that ensures the most appropriate mix of skill and experience to best manage the service delivery requirements associated with

this funded service

Reporting requirements are as per clause 7 of the current Core Agreement. Reports are due on a six-monthly basis unless specified otherwise.

The Organisation will establish a data collection system to enable reporting against agreed service activities and performance measures as required by this service plan

SAF,T reserves the right to charge fees for membership as well as its cultural brokerage/training services and to utilise any such funds to promote the cultural safety and wellbeing of Aboriginal children, youth and families in the NT.

:Special-Conditions

SAF,T and DCF recognise the value of an external evaluation of the 'Circles of Care' model, which will be separately funded by DCF and jointly led by SAF,T and DCF as a collaborative venture. The evaluation will be conducted by a recognised research or educational organisation and comprise a process

evaluation by 30 June 2013 and an assessment of client and service outcomes by 30 March 2014. Outcomes of the evaluation will inform the ongoing service

delivery arrangements. DCF will work with SAF,T to develop the evaluation project plan, governance and scope by 30 November 2012

It is agree~ that this Service Plan will be in a place for a period of 2 years, subject to meeting the agreed deliverables linked to:

1. Establishing the services, its infrastructure, procedures and staffing; 2. Operating the services and service delivery, including working arrangements and protocols with DCF and other services; and

3. Evaluating the services, processes and outcomes.

It is agreed that during the period of this Agreement ACYFS Services in Darwin and Alice Springs will be established and operated by SAF, T. As per Core

Conditions approval is not given to sub-contract either service.

It is agreed that the initial year of this agreement will include both service establishment and service delivery elements. It is understood that case

management for families will not commence until October 2012 (Darwin) and January 2013 (Alice Springs). As such, it is it agreed that the number of families

receiving intensive support services during the first year of this Service Plan will be on a pro-rata basis; that is 20-26 families in Darwin and 15-20 families iA Alice Springs.

SAF,T will report on referrals, assessment and client support (client activity) as per the performance measures in this agreement at Item 2 (Service Provision) on a monthly basis for the first 12 months of this agreement. For the second year of operation SAF,T will report on client activity on a quarterly basis.

In the 2013/14 year, the case load of 30-40 families will be the agreed minimum target number for each service location.

DCF and SAF,T agree to develop shared client outcome indicators including client reporting measures by 30 November 2012, and to collaborate on processes

for referral, assessment and support needs as part of operating the ACYFS services, and to support effective case coordination across both organisations.

DCF recognises SAF,T's ownership and intellectual property in relation to the Circles of Care model

Care and Protection of Children (Information Sharing) Amendment Act

SAF,T understands that the Care and Protection of Children (Information Sharing) Amendment Act commenced on 1 July 2012. The new legislation will make

the sharing of information that relates to the safety and well being of a child easier. To achieve this shift in practice, SAF,T will need to ensure that it has

appropriate policies, procedures and record management practices in place to support the implementation of the Act and will ensure that all personal

information is dealt with in a manner which is consistent with the Privacy Laws and any other relevant privacy legislation.

Transfer to Grants Management System (GMS)

The Department of Children and Families and the Department of Health are currently developing and will be transitioning to a new integrated Grants

Management Framework (GMF) which will be supported by a web based Grants Management System (GMS). When the new GMS 'Goes Live' it will phase out

the current Service Agreement Administration System (SAAS) which administers the current Core Agreements, Variation Agreements and Service Plans.

During the term of this agreement SAF,T agrees to negotiate this Agreement in line with the new contract documentation, which include Agreement Details (Part A), Standard Terms and Conditions (Part B) and Specific Conditions of Funding (Part C).
