2014 Changes Federal Updates
Health Care Reform • All individuals will be required by law to have health coverage beginning Jan. 1, 2014• Health plan participants will pay reinsurance tax — (see page 3)• Additional information on Health Care Reform was sent to homes in September
Same-Sex Spouses
• Same-sex legal spouses, regardless of state residency, will be treated the same as opposite-sex spouses for benefit purposes. This means as of Jan. 1, 2014 same sex spouses will be eligible for:
- Pre-tax benefit premiums; no more federal imputed income - Flexible Spending Account and HRA reimbursement - DaVita Health Savings Account contribution
• All spouses are still subject to dependent verification; marriage certificate, affidavit of common law marriage, and other documentation requirements
Domestic Partners
• Domestic partners are still subject to imputed income and dependent verification
• DaVita HSA contributions are still provided to teammates only. Medical expenses incurred for a Domestic Partner may not be reimbursable from these Accounts under Federal law
Cigna PlansCigna PPO Dental • Dental implants will be added to covered benefits and subject to annual plan limits
Cigna — All Medical Plans • Chiropractic out-of-network services will no longer be covered
Cigna Basic PPO and PPO w/HRA Plan
Emergency Room co-pays will increase (all co-pays waived if admitted)
• $150 co-pay for life-threatening conditions/$300 co-pay for non-life-threatening conditions• Deductible and co-insurance still apply after co-pay

Basic coverage will be reduced• Full time: from 1.5 x salary to 1 x salary / Part time: from .75 x salary to .5 x salary• Supplemental coverage for the reduced amount is available without being subject to

Vitality Points Vitality Points is DaVita’s wellness incentive program that encourages health awareness and accountability, while rewarding you for taking action. When participants complete healthy actions they automatically earn Vitality Points, ensuring they pay the base medical premium in 2014.
IMPORTANT: *Incomplete Healthy Actions will appear as a separate paycheck deduction from 2014 health premiums. Points are available to all teammates, spouses and domestic partners participating in a DaVita health plan. If you think you might be unable to meet a standard for a reward under this program, contact VillageVitality.DaVita.com and we will work with you.
