Background• Lingual thyroglossal duct cyst(TGDC)

– Rare form of TGDC (2~3%)

– Symptoms : incidentally detected ~ life threatening respiratory diffi-

culty (mass at tongue, dysphonia, feeding difficulty..)

– Ultrasound, MRI

– Surgical removal

Case – History of illness• F/2 months• C.C : scalp mass• Birth history

– IUP 38 wks, 3.2kg, NSVD– 2 weeks : laryngoscope d/t grunging repiration : normal

• Physical exam– Mass on frontal scalp– Expiratory grunting respiration

– Lingual thyroglossal duct cyst– Hemangioma, frontal scalp

• Postoperative diagnosis– Same as above

• Operation name– Transoral marsupialization of lingual thyroglossal duct

cyst– Excision of hemangioma

– A. Labeled 'thyroglossal duct': Consistent with thyroglossal duct cyst– B. Labeled 'hematoma': Juvenile hemangioma

• POD # 1– NPO

• POD # 2– Milk feeding

• POD # 3– Discharged

Conclusion• Marsupialization for Lingual TGDC

– Successful– Less complex– Lower morbidity

• Sistrunk operaiton– Complex, higher morbidity (tongue, madible splitting, cervical

approach)• Puncture and aspiration

– Higher recurrence rate
