Overview – Interactive Guide to Navigating Healthcare Coverage Transitions

– Provides information on coverage transitions for older adults and adults with disabilities in Illinois

– Designed for professionals (not consumers) – detailed tips for transitioning; no basic overview of insurance options

– Coverage Finder Tool – Table of Contents

– Guide Sections – provide tips/information on transitions into/using the following forms of coverage: • Medicare Only • Medicare with Employer-Based Coverage • Medicare and Medicaid • Medicaid Only • ACA Marketplace • VA Coverage • Federally Qualified Health Centers (for individuals who cannot access

Assume ALL Case Studies are single (unmarried) adults – households of one (1) for income/asset purposes

Case Study 1 - Mary Mary is currently receiving coverage through the Health Insurance Marketplace. Her employer does not offer health insurance.

Age: Mary is turning 65 years old in April 2015.

Citizenship/Residency: legal permanent resident for 10 years

Income: $2,000 per month

Assets: $150,000 in retirement savings

Work History: Mary has worked in a job that pays Social Security taxes for 15 years.

What coverage is Mary eligible for once she turns 65? 8

Case Study 2 - Monique Monique is currently receiving health coverage through the Health Insurance Marketplace. Her employer does not offer health insurance.

Age: Monique is turning 65 in April 2015.

Citizenship/Residency: U.S. citizen

Income: Monique is single and receives $1,400 per month from part-time employment.

Assets: $100,000 in savings

Work History: Monique has earned 20 work credits paying Social Security taxes.

What coverage is Monique eligible for once she turns 65? 9

Case Study 3 - Juan

Juan is currently receiving coverage under ACA Adult Medicaid because his employer does not offer health insurance.

Age: Juan is turning 65 years old in April 2015.

Citizenship/Residency: legal permanent resident for 25 years

Income: $1,230/month

Assets: $5,000 in a savings account (no other assets)

Work History: Juan has worked for 25 years in a job that pays Social Security taxes.

What coverage will Juan be eligible for once he turns 65? 10

Case Study 4 - Joe Joe currently has health coverage through Medicaid. He lives in Chicago, so he receives his Medicaid coverage through an Integrated Care Program plan. Joe is not currently working. He will reach his 24th month of Social Security Disability (SSDI) benefits in April 2015.

Age: Joe is 54.

Citizenship/Residency: U.S. citizen

Income: $850/month in SSDI benefits

Assets: Joe does not have any assets.

What coverage will Joe be eligible for in April?
