How safe were your patients from adverse events?

What strategies did you use to avoid or mitigate error?

We practice good hand washing practices and doubled checked all medications before administering it

Washed my hands regularly and specifically at 5 required places. Tried to think of red flags. Checked with registrars before acting

Kept them in bed with rails up. MVA patient had neck support to prevent any motion injury

But…..

Quite safe. She didn’t do anything hazardous

I didn’t have risky patients so nothing really happened

My patients were safe. To avoid error I spoke to my registrar

So what can we do?
