you understand, agree, and will comply with the

program guidelines.

Competency checklist is your evaluation of the student’s progress.

Six Categories: Pre-trip, Driving, Backing, Paperwork, Special situations, and Technology.

Enter a start date when you start training, and a completion date when the student reaches proficiency.

After the student reaches proficiency on all tasks, you and the student will sign & date the bottom of the form.

Give the form to the upgrade specialist at the time of upgrade

Students will be required to pass a written test to upgrade. Working with the student by going over the test questions, until they are proficient in answering the questions, will mean success on the test.

In the “Upgrade Student HRS Field” enter a Y in either the super solo or team fields AFTER the student completes 85 BTW hours if you want to run super solo or team miles.
