Our Setting - Geauga County, OH 95,000 residents over 408 square miles. Historically rated in top 3 of all Ohio counties for average family income & public health indicators. Fourth best place to raise a family in the US (Forbes 2008). DDC Clinic is in the geographic center of the worlds 4th largest Amish settlement - 16,500+ with estimated population doubling rate of less than 20 years. ~50% of Geauga County’s developmental disabilities are represented in just 15% of our population. Just 30 miles yet a world away from world-class major medical centers.

Patient Assistance Program • Launched in 2008 to help 1 needy family with medication for

seizure disorders. • Program managed by our business office and nursing • Provides much needed navigation services for families • Insures quality of life for area patients • Has grown to save local community health funds considerable

resources. Will serve a projected 95 families in 2015

DDC Clinic now serves more than 725 patient families with comprehensive services in its medical home Our subspecialty partnerships provide for 1175 additional patient visits each year • fulfilling critical community needs • improving the quality of life for patients and families • helping us help the community contain and reduce the cost of

primary medical care

Together with our subspecialty partners, our sustainable medical home • Conserves community health funds • Means fewer families selling homes, farms and businesses to pay for

outstanding medical bills • Reserves community health funds for truly critical services and

procedures

Holmes County Outreach Clinics

• Born of a tremendous community need coupled with strong ties between the Geauga & Holmes settlements.

• Launched in earnest in 2004 with the support of a five- year award from the Robert Wood Johnson Foundation.

• Support was expanded through 2015 to include both patient care and research with five-year combined commitments from the Ohio Development Disabilities Council, Holmes County Training Center, and area parents.

Sustainability is

ultimately defined in the new but growing community-supported New Leaf Center.
