4. Select “Visa”
5. Read the instructions, scroll down and click “START NEW APPLICATION” to proceed to the next step
6. GENERAL DATA: ALL fields must be filled out, not only those marked with an asterisk. Incomplete forms will be
returned;
The information provided by the applicant is used for issuing the VISA: Be very careful when filling out the visa

7. VISA DATA: All fields must be filled out. Under “Form of Exhibition of Travel Documents,” please choose: BY
