MISSION STATEMENTThemissionoftheForensicHospitalistoprovidethehigheststandardofspecialistmentalhealthcaretomentallyillpatientswhohavebeenincontactwiththecriminaljusticesystemandhighriskcivilpatients,whilemaintainingthesafetyofpatients,staffandthecommunity.

VISITSAspartofthesecurityarrangementsandconditionsforenteringtheForensicHospital,allvisitorsmustapplyandgainformalaccessapproval.

Step 1: APPLYING TO VISIT THE FORENSIC HOSPITALAnyenquiriesregardingprofessionalvisitoraccesstotheForensicHospital,shouldbedirectedtothePAtotheServiceDirector,bycalling(02)97003153,betweenthehoursof9:00am–3:00pm,MondaytoFriday.Allnewvisitorapplicationsmuststatetheappropriatebusinesspurpose,proposeddateandtimeforthevisitandsubmitawrittenrequestforapprovaltotheServiceDirector(seecontactdetails).

Step 2: THE APPROVAL PROCESSOncearequestforavisitisreceivedbytheForensicHospitalsecurityandadministrativecheckswilltakeplacebeforeapprovalcanbegranted.Oncegranted,approvalwillbeadvisedbyanominatedForensicHospitalrepresentative.TheForensicHospitalrequiresaminimumoftwobusinessdaysturn-aroundfortheinitialapprovalandonebusinessdaynoticeofanysubsequentvisitbookings.Approvalforeachvisitisnotautomaticandvisitsmaybecancelledatanytimeduetoclinicaland/orsecurityreasons.

Step 3: YOUR FIRST VISITPleasearriveattheSecureFoyeroftheForensicHospitalatleast30minutesbeforeyourapprovedvisittimeforbiometricregistrationandidentificationcheckstobecompleted.Togainaccess,eachvisitorwillberequiredtopresentindividualacceptableProofofIdentity(POI)andcompletethevisitorbiometricregistrationprocess.

a) Provide acceptable proof of identity (POI) • VisitorstotheForensicHospitalmustprovidesuitableevidenceoftheiridentity.

• VisitorsmustprovideatleastonetypeofPrimaryIdentificationdocumentationcontainingcurrentphotoandresidentialaddress(Passportand/orDriver’sLicencearepreferred).

OR

• ThreeformsofSecondaryIdentificationdocument,oneshowingtheperson’scurrentresidentialaddress.

NB:YournameshouldbethesameonbothacceptablePOIdocuments.

Additional identification requirements for visiting Legal Practitioners

• AcurrentpracticingcertificateissuedundertheLegalProfessionAct2004(NSW)andbecurrentlypracticinginNSW,or

• AcurrentpracticingcertificateissuedundertheLegalProfessionAct2006(ACT)andbecurrentlypracticingintheACT,or

• Acurrentvalididentificationcardissuedbythegoverningbodyofanidentifiedlegalprofession:

-LawSocietyidentitycard,or-BarAssociationidentitycard,or-LegalAididentitycard.

‘Secondary Identification’(Threeformsofsecondaryidentificationwithatleastoneshowingcurrentresidentialaddress):

• AcurrentMedicarecard• PublicUtilityrecords(electricity,gas,waterortelephoneaccounts)issuedwithinthelastsixmonthsofthedateoftheintendedvisit

• ElectoralRollregistration•Currentmotorvehicleregistrationpapers•MarriageCertificate•CreditorDebitcardwiththevisitor’ssignature• PensionerConcessionCardoranyothercurrententitlementcardissuedbythegovernment.

b) Register your biometric data (iris scan and finger prints)

Youwillberequiredtoregisteryourdetails.Youriris,fingerprintscanandphotographwillbeobtainedandstoredonasecuredatabasemanagedbytheForensicHospital.Followingyourregistration,youwillbeaskedtostoreallpersonalitemsthatarenotpermittedinthehospital.KeyoperatedlockersareavailableforuseintheSecureFoyer.Youwillthenbeaskedtoproceedthroughthesecurityscanningandx-rayareaandwillbemetbyastaffmemberwhowillescortyou.

Step 4: VISITING THE FORENSIC HOSPITAL, MALABARWhilstinthehospital,astaffmemberwillaccompanyyouatalltimes.Afterthevisit,youwillbeescortedbacktotheSecureFoyerbyastaffmember.
