•We need to maintain data integrity between both VIHA PACS & PDIV•Errors in PACS/Radnet can affect patient care; especially now that reports get finalized and delivered so quickly.

Our Service is only completed when the ordering physician has the CORRECT results to share with the patient and their family/care givers.

•Every completed order appears on the Radiologists Reporting Worklist and is ready & waiting to be

dictated.

•The goal is to:• minimize errors that can affect patient care • minimize errors that further complicate the downstream PACS and Radnet process • maximize proficiency and accuracy

•Positive Patient Identification•Checking the Physician’s Req. and comparing it to the Cerner Req. for accuracy•Verifying correct order•Verifying marker placement prior to sending to PACS•Verifying images within PACS for correct information•Only completing cases once these checks have been made and all is OK•Ensuring that the Primary Tech does the “Exam Management” portion of the exam – Team approach is error prone

•Use the “Technologist Troubleshooting Workflow” document to determine the course-of-action•A service call may be required•An URGENT service call should be placed for:

•Wrong markers•Wrong orientation•Images on wrong patient

•In many cases a discussion with the reporting Radiologist is a good idea to ensure no incorrect exams are being reported

The Radnet/PACS team has created an online Technologist Resource Document that is accessible from every MI workstation via the MI Main Wiki page.

It is designed for Techs to use as an ongoing reference and troubleshooting guide.
