As required by Section 9-129 of the New York City Administrative Code, I am submitting toyou for transmittal to the Mayor the attached report on out implementation of Sections 9-127and 9-128 regarding discharge planning services. Additional information regarding theDepartment of Correction's Discharge Planning initiative was provided in the Mayor'sManagement Report for fiscal year 2006.

Discharge Planning ProvisionsOctober 2006

Mandate ImnlementationThe Department of Correction (DOC) and the Department of HomelessServices (DHS) have developed and refined a data match process bywhich DOC sends DHS monthly discharge files and DHS thencomputes the percentage of shelter entrants with DOC stays in the sixmonths preceding shelter entry. This data match will be conducted asfrequently as is needed, is currently being conducted monthly, and theresults used in various elements of the discharge planning effort.

9-127-a The Department ofCorrection and the Departmentof Homeless Services shalldevelop a process foridentifying individuals whorepeatedly are adnrltted to citycorrectional institutions andwho, in addition, eitherimmediately before theiradmission to or after theirrelease from such institutionsare housed in shelter providedby the Department of HomelessServices.

Form 983, the Discharge Plariiring Screening form, is completed forevery individual admitted to DOC. Form 983 addresses housing,employment, sobriety, benefits, and family issues. Social serviceorganizations under contract with the Department of Correction arerequired to consult the Discharge Planning Screening Form for eachclient they serve and to document its use.

A prel~ analysis was recently conducted from a sample ofDischarge Planning Screening Forms in order to gauge the social serviceneeds of the sentenced population. A discharge planning database isbeing developed, which will allow all responses to be automated in thefuture, allowing for much more efficient use of the data being collected.Specifications for this database are currently being fmalized.

9-127-b The Department ofCorrection shall collect, fromany sentenced inmate who willselVe, after sentencing, ten daysor more in any correctionalinstitution, information relatingto the inmate's housing,employment, and sobrietyneeds. The Department ofCorrection shall, with theconsent of the inmate, providesuch information to any socialselVice organization that isproviding discharge-planningselVices to such inmate undercontract with the Department ofCorrection.9-128-a The Department ofConection shall makeapplication for governmentbenefits available to inmates byproviding such applications inareas accessible to inmates incity correctional institutions.

Twenty "Benefits Boards" containing applications for benefits availablethrough the NYC Human Resources Administration (13 forms), theSocial Security Administration (I package), and the Veterans'Administration (2 forms) have been installed in ten DOC facilities. TheBenefits Boards also include resources from:the Department of Motor Vehicles (3 forms), the NYC Department ofHealth and Mental Hygiene (5 forms), the NYC Department ofEducation (1 form), US Citizenship and Immigration Services (1 form),and 311 Jail Release Services (English/Spanish flyers).

The Benefits Boards are located in all law libraries and counseling areaswhere they are accessible to all inmates, both sentenced and detained.

111,950 forms and packages have been made available for distributionsince the inception of the Benefits Boards in 2005.

Assistance is being provided through the Department's dischargeplanning contractors. This mandate has been included in the reviseddischarge planning forms used by service providers and in all dischargeplanning contracts.
