Foreword The National Institute of Health and Family Welfare (NIHFW) has a mandate to promote development of a system for continuing in-service training for H&FW personnel at different levels thus contributing to the bigger goal of human resource development. Hence NIHFW has attempted to assess the status of Health and Family Welfare Training Centres (HFWTCs) over the years. Health and Family Welfare Training Centres have been established in the country with the objective to improve the quality of services by providing in-service orientation training to the medical and para-medical personnel engaged in the delivery of health and family welfare services. There are 44 Health and Family Welfare Training Centres funded by the Department of Family Welfare, Government of India functioning in the country. The present study has been undertaken in 2009. Structured formats for collection of information were sent to HFWTCs to assess the current status of their infrastructure and to identify the existing gaps so that steps may be taken to strengthen these training institutions. This report gives the findings of the present study and the recommendations are based on the findings. It has been observed that some of the salient findings and recommendations made earlier are relevant even today. In 1991 NIHFW undertook a countrywide study to review the existing status of Health and Family Welfare Training Centres (HFWTCs). Four regional workshops were organized at Hyderabad, Kolkata, Lucknow, Nagpur and one at NIHFW New Delhi during 1991-92 to report on functioning of Health and Family Welfare Training Centres and formulated proposals for their strengthening. The report was published in 1991-92 for the states of Andhra Pradesh, Kerala, Haryana, Himachal Pradesh, Delhi, Madhya Pradesh, Maharashtra, West Bengal, Assam, Meghalaya, Bihar, Gujarat, Rajasthan, Orissa and Karnataka. This study was preceded by study in 2001 in which information of HFWTCs and RTCs was collected. The report was preapred based on the information collected on structured format sent to the HFWTCs.

EXECUTIVE SUMMARY
To train various categories of health personnel to perform their job functions effectively in implementing the various health programmes successfully, the Government of India started HFWTCs at the rate of 1 per million population. Thus, NIHFW undertook a desk review of the status of infrastructure and training undertaken by 47 HFWTCs which are funded by Department of Health and Family Welfare, Government of India located in 21 states and UTs. Information was obtained from total of 44 training institutions from 21 states viz: Andhra Pradesh (4), Delhi (1), Gujarat (1), Haryana (1), Jharkhand (1), Kerala (1), Karnataka (2), Maharashtra (4), Meghalaya (1), Madhya Pradesh (3), Orissa (2), Rajasthan (2), Tamil Nadu (3), Uttar Pradesh (7), Bihar (3), Himachal Pradesh (1), Punjab (1), J&K (1), Chhattisgarh (1), Manipur (1) and West Bengal (3). Three of the 47 HFWTCs (HFWTCs, Thycaud, Kerala; HFWTC Guwahati, Assam and HFWTC Ahmedabad, Gujarat) were excluded from the study as they have been upgraded to SIHFWs. Most of the large states have 1-7 HFWTCs, whereas North-East states like Manipur and Meghalaya have one HFWTC each.
Infrastructure
Most of the HFWTCs have good infrastructure facilities with adequately furnished classrooms and library with good number of books. 39 institutions have hostel facility but only 16 institutions have mess facilities available round the year. HFWTC, Kolkata, HFWTC, Jalpaiguri, HFWTC, Delhi and RHFWTC, Patna do not have hostel facility. Only 13 HFWTCs are located within the hospital premises. Majority of the training institutions have their own vehicles for transportation of trainees except HFWTCs, Bhagalpur, Muzaffarpur and RHFWTC, Shillong. • Provision of Classrooms and Audio Visual Aids The AV teaching aids are available in most of the institutions except HFWTC, Muzzafarpur, HFWTC, Kolkata and HFWTC, Nadia. Over Head Projector is available in 36 centres, LCD in 26 and desktops in 24 centres. Laptop is available in only 12 HFWTCs. Although most of the training institutions have adequate infrastructural facilities, there may be need for upgradation/modernization so that appropriate A/V aids including LCD projector be provided in all the training institutions. A part of the fund under NRHM could be sanctioned as administrative

expenses for the above purpose. Alternatively, a room and equipment may be provided either in the training institution or at the district hospital. The LCD projector facilities would be helpful in taking theory sessions as well as demonstrating the procedures like measuring weight, BP, IUCD insertion etc., prior to the trainees practicing the techniques in the OPD and wards of the district hospital using highly skilled human resource at the state level.
• Manpower The manpower available in the various institutions, varies enormously both in terms of numbers and in terms of area of specialization e.g. 16 centres have one sanctioned post of PHN/PHNI, 20 centres have 2 posts, 2 centres have 3 posts, 1 centre has 4 posts, 1 centre has 6 posts and 3 centres have not given the sanctioned posts of PHN/PHNI. There are lot of vacancies in the manpower of the training centres. Sixteen centres do not have the post of MLCD in the institute and 6 centres do not have Principals. There is lot of variation in the nomenclature of the staff positions like some of the centres have a post of Sanitary Engineer and some have Senior Sanitarian post for Sanitary Inspector. • Appropriate Placement of Manpower/Human Resource The gaps in human resource in the various HFWTCs may need to be appropriately filled. The human resource should have qualifications, experience and skills as would be needed for providing training especially skills for those peripheral health personnel who are posted below district level. Training Most of the HFWTCs undertake about 6-8 training courses per year mostly of theoretical type. They may be required to coordinate skill-based training for all services that need to be provided for the health of the population. In addition they may have to coordinate/undertake training pertaining to administration and management of the health care facilities and the health system.

Review of HFWTCs
Soon after attaining Independence India undertook the mammoth task of creating the health care infrastructure to meet the health care needs of the vast population of India. This included setting up of medical and nursing colleges for pre-service education and training institutions for undertaking in-service training. When the HFWTCs were created with a norm of 1 HFWTC for one million population in the sixties and seventies there was very little peripheral health care infrastructure; PHCs, sub-centres and the manpower thereof etc. had not been fully developed. The district hospital, medical college hospital and few sub-district taluk hospitals provided health care. Outreach if any were provided by the uni-purpose workers for vertical programme e.g. malaria, leprosy, family planning etc. Since the hospitals had well trained doctors, nurses and other paramedical staff, the peripheral workers were trained by programme officers of each of the vertical programmes, the major role of the HFWTCs undertook was dissemination of knowledge regarding family planning, MCH and some health programmes like tuberculosis control, etc., their staffing pattern and the other infrastructures were developed to support this role.
By the 1980s India established a vast infrastructure for peripheral health care
including manpower for integrated outreach services in each of the villages. The team leader for delivering health care services (preventive, promotive and curative) at the peripheral level is the Medical Officer PHC (MOPHC). He/She has a team of Multipurpose Health Assistants/Supervisors (M&F) and Multi-Purpose Health Workers (Male & Female), who are the most peripheral outreach health care providers and in addition there are staff nurses, lab technicians and some support staff (facility-based) for providing the health care services. All of these people need technical, communication and managerial skills including skills for supportive supervision for their subordinates to enable them to provide good quality integrated health and nutrition services.
India is also currently undergoing demographic, health and nutrition transition
and hence having dual burden in many areas; increasing non-communicable diseases with no reduction in maternal and child morbidity and mortality as well as in communicable diseases; also problems of under-nutrition as well as over-nutrition. There has been some marginal decline in children under the age of 14 years; with increasing longevity of population >60 years.
Given these factors, the HFWTC and other training institutions need to ensure
skill up-gradation of all in-service personnel especially peripheral health care so that they can provide good quality health care services in the current situation. NIHFW undertook a review of status of HFWTCs in the country with regard to manpower, AV Aids, hostel facilities, location vis-à-vis a well functioning medical college or district hospital and the kind of training undertaken by the centers in the last few years.

I. Salient Findings
A. Current role and training undertaken by the HFWTCs Health and Family Welfare Training Centres have been established in the country with the objective of giving in-service training to health personnel in the rural health sector. These training centers are set-up with 100 per cent financial assistance from the central government. There are 44 HFWTCs in the country at present. HFWTCs have been giving basic training to MPWs (M) as well, from 1982. The HFWTCs also take up in-service training under various vertical national programmes. At present HFWTCs are providing in-service training under Reproductive and Child Health Programme. Most of the courses they undertake are knowledge-based courses; this includes some TOT courses for ASHA recently. In some states like Tamil Nadu and Maharashtra, HFWTCs undertake coordination of/conduct skill upgradation courses.
B. Availability of Human Resources
There is tremendous variation in the type of staff and number available in the HFWTCs.
(i) Technical Staff Table 1.3 shows that 6 HFWTCs do not have a Principal posted currently. Of 44 institution, post of MLCD in 11 institutions the post is lying vacant. Two HFWTCs have 2 posts of MLCD sanctioned out of which 1 is filled and other is vacant. In the case of PHNI the range of number of sanctioned post is 1 to more than 4. In 24 institutions all posts of PHNI are filled, 16 institutions have vacant posts and 3 institutions do not have sanctioned post for Public Health Nurse Instructor. Twenty Eight HFWTCs have 1 sanctioned post of Social Science Instructor, of these the post is filled up only in 6. Five HFWTCs have 2 posts of Social Science Instructor, out of these 4 centres have vacant posts. Ten HFWTCs do not have any sanctioned post of Social Science Instructor. Eighteen centres have 1 sanctioned post of Sanitary Inspector out of which 14 posts are vacant. Similarly, 7 centres have sanctioned post of 2-4 of which 6 are vacant and 18 centres do not have sanctioned post.

(ii) Support staff Clerical staff: The sanctioned posts of clerical staff range from 1-10. One institution has more than 10 sanctioned posts whereas 2 centres sanctioned posts of clerical staff. Twenty five centres have all posts filled and 16 centres have vacant posts. Absence of clerical support staff will make it difficult for conducting courses as the faculty will need to undertake all the ministerial tasks also in addition to actual conduct of the training courses. Grade IV staff: Three institutions do not have any sanctioned post of Grade IV staff. In 19 centres more than 10 posts are sanctioned, out of these only 4 centres have filled all the posts. The remaining 21 centres have 1-10 sanctioned posts, of which only 7 centres have filled all the posts. Without adequate number of Gr. IV (Support Staff), the trainees are facing inconvenience while residing in the hostel due to lack of support services. Drivers: Two institutions have no sanctioned post of driver. The sanctioned post of Driver in the other 41 institutes range from 1-3. Nineteen institutions have filled all the posts of drivers whereas 22 institutions have vacant posts. Absence of drivers will limit the institutions from taking the trainees to hospitals for skill training. Others: Many of the centres have several other sanctioned posts like Epidemiologist, Sub-Divisional Engineer, Statistician, Artist-cum-Demonstrator, Communication Officer, Management Instructor, Accountant, Dy. MEIO, Health Extension Educator, Maternal Child Health Officer, Health Education Instructor, Sr. Health Inspector; many of these posts have been lying vacant for more than 10 years.
C. Provision of classrooms, teaching aids and Library facilities • All HFWTCs have adequate number of classrooms.

• OHP is available in 36 of HFWTCs (8 institutions indicated non-
functioning of OHP), LCD is present in 26 of the centres and 24 have desktops but laptop is available in only 12 HFWTCs (Graph-2).

• While 35 training institutes have mess facility in the training centre; only 16
institutions have mess round the year and the remaining have mess available only if there is any training. Nine HFWTCs do not have a mess at all (Graph-5).

• Other amenities like water/electricity have to be provided so that the participants can stay in the HFWTCs. HFWTC Jabalpur, MP has expressed the need for getting boring done in order to have sufficient water in their training centre.
• Absence of hostel mess and other amenities will have an impact on the use of
the HFWTCs for training as well as in the type of courses that can be conducted in such centres. Lack of such facilities will prevent proper residential training as participants will cite the lack of basic amenities and not stay in the hostel.

Number of trainings and availability of mess and hostel As can be seen from the Graph-6, while the availability of mess and hostel has certainly helped institutions to conduct more courses, the absence of hostel and mess has not prevented some institutions from conducting trainings. There are also few institutions who inspite of having both hostel and mess have not conducted any training (Graph-6).
Graph-6 E. Transport availability • Vehicle is available in 41 of the training institutes, out of which 22 centres have 1-
2 vehicle, 15 have more than 2 vehicles while 4 did not give the exact number of vehicles on the road. Three institutions do not have a vehicle i.e. HFWTC, Bhagalpur, HFWTC, Muzafarpur and RHFWTC, Shillong.

• Even in those with adequate number of functioning vehicles, the use was limited
due to inadequate POL budget and hence there is difficulty in taking trainees to the hospitals or field areas for training (Graph-7).
F. Linkage with hospitals/health care facilities
• The linkage of HFWTCs with hospitals/health care facilities is very poor.
Only 13 HFWTCs are located within the campus of hospital, and another 7 are located within 1-2 km from the hospital; 11 centres are located more than 5 km away from the hospital and 12 of them are located within 2-5 km. (Graph-8). In case the HFWTC is far away from the hospital, posting of trainees in the hospital especially round the clock for skill up-gradation becomes very difficult.

As can also be seen from the graphs availability of staff and teaching aids (Graph-9 and 10) has facilitated conduction of more trainings but absence or limited availability of either or both has not deterred some institutions from conducting trainings. Similarly in a few instances inspite of availability of all facilities and staff the institutions have not conducted any/adequate number of trainings. Admittedly in all the above four cases the quality of training would have suffered in many ways and hence it should be prevented; those institutions in spite of limitations are trying to conduct training must on priority be provided with all support infrastructures.
• HFWTC Nadia doesn’t have any teaching aids but conducted 8 trainings since last three years.
• RHFWTC Kurnool, RHFWTC Bilaspur and RTC Vishakhapatnam have more than 3 teaching aids but conducted only 4 trainings.
• RHFWTC Patna didn’t conduct any training although it has more than 3 teaching aids.
• HFWTC Kozikode, HFWTC Madurai, HFWTC Egmore, RHFWTC Dharwad, RHFWTC Gulbarga, RHFWTC Kangra, HFWTC Amritsar and RHFTC Shillong have less than 3 teaching aids but conducted more than 15 trainings.
• RHFWTC Bhagalpur has both hostel and mess facilities but did not conduct any training in last three years. Similarly HFWTC Varanasi and RHFWTC Gorakhpur have both facilities but conducted less than 5 trainings.

• HFWTC Egmore has 60% technical posts vacant but conducted 16 trainings likewise
• Jhansi has 83% vacant posts but conducted 12 trainings.
• RFPTC Kangan and RHFWTC Bilaspur have only 33.3% vacant posts and hence conducted only 3-6 trainings.

Recommendations Based on the findings of this review as well as earlier reviews of HFWTCs conducted by the NIHFW in the last 15-20 years as well as our experience in coordinating in-service training in the country under IPP, RCH-I and NRHM, following is suggested:
I. Rationalisation of number and role of HFWTCs Given the increase in population as well as huge increase in health care infrastructure especially peripheral health care personnel and the need to provide comprehensive health care services for the population, there may be necessity to review the norms for HFWTCs in terms of the number/state, their role and liaison with:
State level training institutions like SIHFW, ANMTC, DTC.
Tertiary health care facilities like, Medical College Hospital, District Hospital, as well as FRU, PHC and Sub-centre.
Linkage with health care facilities would be required since the health care services providers in the tertiary and secondary care level would be the ideal trainers to provide skill upgradation training for peripheral health care workers.
Number of HFWTCs • The existing population norms of 1 HFWTC/10 million population may
still be retained. However the total number of available training centres at above district level in each state HFWTCs/Regional HFWTCs/RTCs/Divisional Training Centres need to be enumerated and shortfall if any need be corrected by creation of new HFWTCs located in the division or areas where there are none. The creation if any for each state may need to be in accordance with current (2001 Census) population; in certain areas like the North-East a HFWTC may be given for a group of states.
Proposed role of HFWTC in In-service training • Coordination of training especially for the MO PHCs (all administrative
arrangements for successful conduction of training e.g. ensuring letters are sent on time, ensuring early and appropriate nominations, payment of TA/DA, ensuring stay and transport where needed etc) as well as liaison with hospitals and some support at the hospitals so that the trainees do get to practise the skills.

• The faculty posted at HFWTCs need to develop liaison with the Programme Management Support Unit (PMSU) as well as other district level personnel of Dept. of WCD, PRI, AYUSH and Dept. of Drinking Water and Sanitation in order to orient all on the mechanisms of implementing convergence in provision of services and local ownership and oversight of the programmes. The linkages with PMSU will also help the HFWTCs to institutionalise planning for training from sub-centre upwards and have an updated database of trained personnel as well as the current availability of qualified and/or trained personnel in the various health facilities in a district.
Funding to HFWTCs and ANMTCs • Since HFWTCs are centrally sponsored institutions, it is recommended
that the funds to these institutions may be routed through NIHFW instead of State treasury. This will not only ensure that these institutions will get the funds on time, but also NIHFW can monitor the quantity and quality of training being imparted by these institutions.
• NIHFW can develop norms for rationalization of the funding to HFWTCs and ANMTCs along with Objectively Verifiable Indicators for inputs and processes and perhaps for outcomes (like number of skill-based training conducted, no. of faculty whose skills have been upgraded and no. of functional equipment and the duration of the equipment being not functional).
II. Rejuvenate Training Package
Human Resource • There may be need for rationalisation of the technical manpower available at
HFWTCs; both type of staff as well as number of each type. The rationalisation needs to take into account the type of trainings both in-service and may be some others envisaged in the HFWTC in the next 10-20 years.
• In case it is envisaged that they take an active role in skill upgradation in-
service training of MOPHC there may be need for appropriately qualified faculty/staff e.g. Preventive and Social Medicine, Nursing Tutor/PHNI, expertise in Communication; similarly need for staff with expertise in accounting, MIS and Finance may be required as the HFWTC need to also orient/train the MOPHC in these areas; the HFWTC can also take up training of PMSU at block level. The staff at the HFWTC will need to have linkages with the specialists posted at District Hospital/Medical College Hospital and FRU (especially Obstetrics & Gynaecology, Paediatrics, Medicine and Surgery) so that the trainees get adequate exposure in the facilities for practice of the skills.

Need to provide minimum posts at each HFWTC
It is recommended that there should be minimum posts at each HFWTC to
prevent the disparity of number and category of staff. Apart from technical and support staff 10 consultants of various fields should be appointed at each HFWTC which are as follows:
o Consultant (MCH) o Consultant (Nursing) o Consultant (Public Health) o Consultant (IT) o Consultant (Social Science) o Consultant (Management) o Consultant (Statistics/Demography) o Consultant (Environment Health) o Consultant (Training Education) o Consultant (Communication)
• All the above consultants and the specialists at the medical college
hospital/district hospital will need upgradation of their knowledge at periodic intervals so that they can ensure appropriate skill and knowledge upgradation of their trainees.
• Similarly there is a need for rationalisation of support staff for HFWTC, it is
suggested that in place of technical assistant or computer assistant, skilled person should be appointed who is efficient in statistical package, accounts software and data entry. The office support staff need to be proficient in use of modern technology like computers. Outsourcing of some of the services like housekeeping, kitchen, security, horticulture, cleaning etc using mechanisms already being practised in the concerned districts may be considered.
• All the faculty and staff should be involved in the training programmes and
they should be encouraged to take up research work other than regular training activities.
• Frequent transfer of faculty staff should be avoided and vacant positions of
various categories may be filled-in as early as possible.
• Those institutions inspite of limitations are trying to conduct training must on priority be provided with all support infrastructures.

Infrastructure Provision of Teaching Aids and expansion of Seminar room/Classroom • Effort should be taken to increase the number of classrooms especially in
those HFWTCs which have only 1-2 classrooms and undertake renovation wherever required. It is also suggested to expand the seminar room/classroom to accommodate more participants at a time.
• There is the need to strengthen teaching and training aids like computer
(desktop or laptop, mike and LCD projectors etc) in the HFWTCs in almost all the states. A part of funds under NRHM could be sanctioned as administrative expenses for the above purpose.
• All of the HFWTCs should have video conference facility that would be useful
in taking theory sessions as well as demonstrating the procedures like measuring weight, BP, IUCD insertion etc., prior to the trainees practicing the techniques in the OPD and wards at the district hospital.
• All the HFWTCs need to have a good library with standard textbooks as well
as copies of all guidelines for all health and family welfare programmes.
• The HFWTCs should have computer lab with internet connectivity so that the staff of HFWTCs as well as the trainees can access and download information/guidelines, monthly performance reports etc. pertaining to programmes for use of themselves/trainees.
• To strengthen the health information system, it is recommended that each
HFWTC have their own website so that necessary information could be availed. Apart from this, database could be created on the website about the trainings and the participants.
III. Provision of Residential Facilities
• Provision of facilities for stay at clinical training site
Since the focus is on skill upgradation, there has to be provision of at least 2 rooms/dormitory with basic amenities in district hospital/sub-district hospital/FRU (functioning) to accommodate 2-3 persons so that the trainees can be posted round the clock in the hospital. This will not only help the trainees to practice the skills required at the clinical training site with supportive supervision/mentoring by the trainers; a spin off benefit for the hospital is that the hospital staff have some trainees who will share in the workload in OPD, labour room etc.

A part of the funds provided under NRHM for upgradation/maintenance of DH/FRU/SDH may be used for the purpose, especially in those places where the training institutions are located far from the hospital.
• Provision of residential facilities
It is suggested that adequate and proper residential facilities (both boarding and lodging) are required in all the HFWTCs for the trainees so that the training can be truly residential. The hostel/quarters in district hospitals if available could also be explored for use of trainees.
• Renovation of guest house/hostel
The guest house and hostel should be renovated and furnished to provide ease and comfort while stay during training. Computer with internet facility could also be provided in the guest house for the participants.
• Those institutions inspite of limitations are trying to conduct training must on priority be provided with all support infrastructures.
IV. Other Infrastructures
• Transport facilities
It is recommended that either adequate provision be made for transport of trainees to the hospitals/FRU/PHC etc and/or the HFWTCs may be permitted to outsource for vehicles using the mechanism that the districts are using.
• Linkage with FRU, PHC and sub-centre
Every HFWTC must have linkage with a functioning FRU, PHC and sub-centres in view of a possible wider role in orienting new entrants like the PMSU for demonstrating the infrastructure as well as the functioning of the peripheral health care facilities and manpower.
V. Capacity building of faculty
In order to successfully undertake the task of training the MO PHC the trainers at the HFWTC as well as the medical college faculty and DH specialists, need to have the knowledge of programme including changes in the programmes and the skills for which they should be providing training. They (trainers) need to be practicing the skill in the hospitals as a routine. Supplies and functional equipment need to be made available to operationalise health care facility where trainee will work after training. In addition there is need for upgradation of the knowledge and skills of the functionaries in computer applications, accountancy, MIS etc.

• Diploma/Course in Public Health should be started at each HFWTC as a medium to generate revenue.
• All faculty members should be encouraged to engage in research work in addition to regular classes.
VI. Role of CTIs in supervision of HFWTCs
The faculty of CTIs should visit the HFWTCs frequently for evaluation and guidance of these institutions. They need to monitor, do handholding and play a role of mentor, wherever necessary. Training curricula may be prepared with the help of CTIs.
VII. Review Meeting
It is recommended that a three day Regional Review Meeting be conducted at NIHFW with the Principals and Faculty of HFWTCs in order to finalise the minimum manpower, infrastructure requirements as well as the human resource issues.
