Health Instability and Medical Complexity (CHESS)

Changes in Health, End-stage disease and Signs and Symptoms

(CHESS)

Scores range from 0 to 5 - CHESS measures medical complexity and health

instability. It is based on: vomiting, dehydration, weight loss, shortness of

breath, edema, end-stage disease, decline in cognition and ADL, ate fewer

meals, decrease in fluid or food, and fluid output exceeds input. Higher scores

indicate higher levels of medical complexity.

Score Description

0 Stable

1 Low level of medical complexity and instability

2 Mild level of medical complexity and instability

3 Moderate level of medical complexity and instability

4 High level of medical complexity and instability

5 Very high level of complexity and instability

Depression

Depression Rating Scale (DRS)

Scores range from 0 to 14 - The DRS is based on

seven items: negative statements, persistent anger,

expressions of unrealistic fears, repetitive health

complaints, repetitive anxious complaints, sad or worried

facial expression, and tearfulness.

Score Description

0 No symptoms of depression

1-2 Some symptoms of depression –

intervention may be helpful.

3-5 Possible depression

>6 Possible severe depression
