• One form display for all forms is better for parents - navigation wise (or you can choose to have all paper forms into one form)

• Parents fill out online forms right away or forget about them

• You will need to send reminders

• You can limit who can see forms - only parents of rising K, 7 and new students need to fill out the immunization form

• You can add html into form text boxes and the web browsers will read

and display it correctly. This helped us color text, bold text, create

bulleted lists, and insert links within a form itself.

• You can hide the steps at the top if you need to - submit a case to

Blackbaud and they will edit the CSS for you (I do not recommend

editing this on your own if you don’t know CSS)

• Online Admissions forms can have logic built into them (hide

sections, steps based on 2013-14 grade)

• Paper back up for parents without internet access

- Computer on campus for parents to fill out forms

- Library/conference room/kiosk by front doors are some ideas

• Roll out

- Notify parents/constituents well in advance of change

- Be sure to pilot with a small group and solicit feedback!

- Feedback is invaluable!
