INTRODUCTION

WHO ARE YOU, AND…?

WHY ARE YOU HERE?

To learn how the MODIFYwith CED Program can help you as you transition into adulthood.

THE PURPOSE OF THE MODIFYWITH CED

PROGRAM

To ensure that children who are likely to remain in foster care until 18 years of age are provided education, training, financial support, and other needed services.

To support and serve former foster care recipients between 18 and 20 years of age to complement their own efforts toward self-sufficiency, and to assure that program participants recognize and accept their personal responsibility for preparing to make the transition from adolescence to adulthood.

PAYING FOR YOUR HIGHER EDUCATION

3 WAYS TO PAY: WAY #1

Covers tuition (the cost of classes)

Does NOT cover room and board, the cost of books, or other fees

MUST complete the FAFSA in order to receive

3 WAYS TO PAY: WAY #2

FAFSA

Free Application for Federal Student Aid To be completed @ www.fafsa.ed.gov Question: “At any time since you turned age

13, were both your parents deceased, were you in foster care or were you a dependent or ward of the court?” Answer: YES!!!

Deadlines: January 1st –FAFSA available for completion March 1st – Deadline to apply for: the PROMISE

Scholarship; the WV Science, Engineering & Technology Scholarship; and the Underwood-Smith Teacher Scholarship

April 15th – Deadline to be considered for the WV Higher Education Grant

3 WAYS TO PAY: WAY #3

Up to $5,000/year Can be used to cover the costs of attending college or

vocational training that are NOT covered by the tuition waiver or the funds received from completing the FAFSA May include the cost of a computer, special clothing, books,

housing, transportation, etc.

If all educational expenses are covered, may be broken into monthly payments

Chafee Education and Training Vouchers (ETV)

ELIGIBILITY CRITERIA In order to receive the tuition waiver, youth MUST:

have been in foster care for at least 1 year prior to the waiver application;

have graduated from high school or obtained their GED while in the custody of the DHHR;

have been accepted to a WV public higher education institution; AND

complete and submit the FAFSA. In order to receive funding available from completing the

FAFSA, youth MUST: be attending a post-secondary educational program.

In order to receive Chafee funded ETV, youth MUST: have been discharged from foster care after the age of 18; OR have been adopted from foster care after the age of 16; OR have been placed in legal guardianship; AND be attending a post-secondary educational program.

THE FC-18 - WHAT IT IS & WHAT IT IS NOT

It is NOT placing yourself back into the “custody” of the state. It IS a “contract” between you and the Department. By signing it…

You have the following responsibilities: To work toward meeting the goals and objectives of your transition plan. To work on becoming semi-independent and self-sufficient.

The Department has the following responsibility: To provide foster care services, including financial assistance and a

continued medical card so as to meet the goals & objectives of the transition plan.

It will be terminated if you… Voluntarily decide to no longer participate in the MODIFY program Attain the age of 21; Discontinue regular participation in an approved educational

program; Demonstrate an unwillingness to cooperate with the DHHR or the

MODIFY Program; OR Violate any law of any State, County, or City of the United States

that results in you being incarcerated.

SERVICES THAT REQUIRE THE FC-18

Monthly Subsidy $900/month if residing in an apartment. $200/month if residing in a dorm.

Medical Coverage Medicaid coverage is continued until the

age of 21. Start-up Funds (one time only)

Up to $1,100. May be used to purchase dorm necessities. May be used to pay a security deposit

and/or first month’s rent. May also be used to purchase household

supplies, food items or to pay for necessary utilities.

REQUIREMENTS FOR MODIFY SERVICES

WHAT DO YOU HAVE TO DO?

Be employed with an educational plan Be enrolled in an educational program Be engaged in at least 40 hours/week

of productive activity May include attending school, working,

volunteering, etc. If you are a full-time student, then you do

not need to participate in any additional productive activity.

Submit your monthly budget and productivity report (if applicable). MUST be received in order to process

your monthly subsidy.

WHAT DO YOU HAVE TO DO? (CONTINUED)

Provide your MODIFY Worker with the following information: Valid address, phone number, & email.

You MUST update your MODIFY Worker if this information changes!!!

Name and phone number of an adult contact.

User ID and password for your school account (if applicable). Allows your MODIFY Worker to check your

class schedule, check your mid-term and final grades, and make payments on your account.

You MUST update your MODIFY Worker when your password changes!!!

WHAT DO YOU HAVE TO DO? (CONTINUED)

Provide your MODIFY Worker with the following documents: Copy of your high school diploma or GED Copy of your acceptance letter to a post secondary

institution Copy of your FAFSA completion letter

Immediately after submitting your FAFSA online, this will appear on your screen.

WHAT DO YOU HAVE TO DO? (CONTINUED)

Complete 80% of your coursework

Maintain a 2.0 GPA each semester

Have monthly contact with your MODIFY Support Specialist. Can be via email, text, call, or

visit.

NOT MEETING REQUIREMENTS

PROBATION If you do not maintain a 2.0 GPA, you will be

placed on probation. The probationary period will last for 1 semester. You and your MODIFY Worker will create an

Educational Improvement Plan that you MUST abide by in order to remain in the program. It will include the following: You MUST bring your GPA up to a 2.0 by the end of that

semester, or you will be discharged from the program. If by the mid-term of your probationary period you are

failing any classes, you will be required to attend tutoring.

AFTER PROBATION

If you do not bring your GPA up to a 2.0 by the end of your probationary period, you will be discharged from the program.

BUT…

You have the option to continue attending school, paying for it yourself; AND

If you bring your GPA up to a 2.0, we may allow you back into the program if it is at least 1 semester before your 21st birthday.

EXCEPTION

You will NOT be offered probation if you achieve a 0 GPA!
