Background
Major accidents are often said to
be ”Black swans”, surprising
events that are not possible to
foresee.
Det norske’s approach is based
upon an idea that it is always
possible to detect negative
developments within an
organization and do something
with them before an accident
occurs.
Major accident/process
indicators are developed for
production facilities.

Development
Autumn 2008: In house development of indicators in a multidisciplinary
team, based on experience, theory and accident investigations.
The indicator should be based upon existing data acquisition.
2009: The major accident risk indicators implemented at Bredford Dolphin,
Songa Delta and Aker Barents.
Winter/spring 2010: Project to further develop indicators in cooperation with
SINTEF.
Changed some indicators, replaced other.
If possible, tried to adjust indicators to fit the RNNP-reporting.
Revised indicators implemented on Aker Barents and Songa Delta in October
2010.

Are we measuring the right issues?

Experience
The major accident risk indicators have contributed to raised awareness to
the issue in Det norske and among our contractors.
Have got more information about and set focus on some issues, for example
maintenance and hot work.
Numbers are not enough – we need some qualitative explanation as well.
Large variations in normal level for the indicators, depending on rig/project.

Challenges / Further development
Ownership and commitment to major accident risk monitoring throughout
the organization.
Validity of the indicators – are the indicators valid measurement for major
accident risk.
Reliability of the indicators – how to define the indicators good enough to get
the same reporting from different projects.
Define acceptance criteria – when to take action.
Further development of indicators for human and organizational factors.
Further development of barrier indicators.
Major accident indicators do not improve safety, it’s just a measurement of
how well our safety work is.
