1. Download Registration Forms Packet
2. Save to desktop
3. Open file
4. Click ‘Save As’
5. Rename file with your student’s name
6. Please print forms SINGLE SIDED
CARMEL VALLEY REGISTRATION CHECKLIST
Mandatory Forms: Please make sure you have completed all the mandatory forms, provided copies of 2 documents
verifying your residence (see residency verification form) and included a copy of the birth certificate and immunization
record.
To expedite the registration process, you must include copies of all required documents. Incomplete packets will not be

FOR EASE OF PROCESSING PLEASE SUBMIT IN THE FOLLOWING ORDER:

Please note the section regarding parent education level and select the highest level of education
achieved by the parent.
►IMPORTANT: When you check Yes or No for Hispanic or Latino for the student’s ethnic background you need to
ALSO indicate the race from the choices provided under race.
2. □ VERIFICATION OF RESIDENCY
Return completed Verification of Residency form along with a copy of the top and bottom of your current electric bill
plus an additional form that reflects your name and current address. See Category 2 on the Residency Verification form
for a list of acceptable documents. Please make sure your primary residence address and name are on the document.
The Residency Verification form and documents must be submitted in order to schedule classes for the 2018-19
school year.

GRADE CLASSES
Welcome to Bobcat Country!
Before beginning the registration process, please read this sheet carefully.
The Course Selection Form is due with the Registration Packet on March 22nd
, 2018.
COURSE SELECTION FORM INSTRUCTIONS:
1. Please PRINT your last and first name neatly.
2. You will automatically be placed in 7th Grade Science.
3. You will automatically be placed in World History.
4. If you are interested the zero period option, please check the box. For information, refer to the Zero Period Option page.
5. You must take PE.
Students who take music may also enroll concurrently in ISPE to earn PE credit (ISPE Application required)
and have the option to enroll in an additional elective.
If you want to take both PE and music, number PE as your number one Physical Education choice and number
Band or Orchestra as your first elective choice.
6. You will have space in your schedule for ONE YEAR-LONG ELECTIVE.
Please make a careful and thoughtful decision based on your abilities and dedication to studies because you will
be making a year-long commitment. The choices you make now will determine your 2018-2019 schedule.
Applications and Teacher Recommendations are required where noted. Forms are available on the Counseling
website (CV Registration Forms link) under optional forms.
A World Language is not required for a high school diploma; however, two years are required to apply to a 4-
year university. At least one year needs to be taken at the high school for all years to count. (For example, you
cannot take just Spanish I and II in 7th and 8
th grade for it to count for college. You will need to take Spanish III
in 9th grade for your middle school Spanish to count). You are advised to begin your language by 9
th or 10
th
grade. You will need to wait until 8th
grade if you wish to study French. You will have to wait until high school
if you wish to study Chinese, Japanese or American Sign Language. NOTE: not all world languages are offered
at all campuses.
Auditions are required for placement in Intermediate or Advanced music classes. Please check the Counseling
website (Registration Forms link) under optional forms for more information on audition registration.
Please complete a Music Intent form if you select Band or Orchestra
7. Check the English course you would like to take. We strongly encourage you to review the English Course Profiles to
help guide your decision. Course Profiles are available on the Counseling website.
8. Check the math class you would like to take. We strongly encourage you to review the Integrated Math A and Integrated
Math A Honors Course Profiles to help guide your decision. Course Profiles are available on the Counseling website. If
your student earns a score indicating he/she should be placed in Integrated Math A Readiness on the MDTP (a math test
administered at your elementary school), you will be contacted by Carmel Valley. If you plan to take the Integrated Math
B Honors Readiness Test, sign up for Integrated Math A Honors and we will adjust your schedule if you meet the
requirements.
A parent signature is required on the Course Selection form.
*NOTE: Schedules will not be made to accommodate teacher requests, period requests or extracurricular
activities. 8th
graders are given preference in elective classes.

No schedule changes will be made to accommodate teacher or period requests, or for off campus, extracurricular or athletic activities.
SDUHSD Board Policy sets a four-week limit at the beginning of each semester to add or drop a class without it reflecting on the
transcript. Level changes (ex. from honors) may be requested during this period only if there is space available.
Courses may be cancelled due to insufficient enrollment or staffing restrictions.
