Smile Train is pleased to offer online Final Grant Reports.

We welcome your comments and questions as

you review the following instructions.

If you prefer to fill out an offline form, please

contact Smile Train’s Country/Regional Manager

in your area to request that the form be e-mailed

to you.

Your Grant Account: Logging In

The e-mail you received - to let you know your Final Grant Report form was

available online - included a link to your Grant Account:

https://www.grantrequest.com/SID_1496/Default.asp?SA=AM&FID=&SESID=23824&RL=

Final Grant Reports: New, In Progress and Submitted

Once you log on, click on the Final Grant Report Tab.

Completing a Final Grant Report – Questions and Attachments

Some Questions and Attachments will be Required; some will be Optional.

Completing a Final Grant Report – Saving a Report

If you do not want to finish and Submit the report right away, you can Save the

report and come back later.

Completing a Final Grant Report – In Progress Reports

After you have saved the report, you can see it listed under In Progress.

If you exit the Grant Account and come back later, you can get back to your

Grant Account by using the same link:

https://www.grantrequest.com/SID_1496?SA=AM

Select the Final Grant Report Tab and In Progress list.

Open the report by clicking on the name.

Completing a Final Grant Report – Submitting

Enter all information, upload Attachments and click on Review & Submit.

Scroll down to the

bottom and Submit.

Sharing a Final Grant Report by E-mail

To share a copy of the report*, you can do so after it is Saved, or after it is

Submitted. *with the Smile Train Country/Regional Manager in your area, or with anyone else you want to send it to
