reprimand placed on his permanent disciplinary record with the Board, paid a civil penalty of $3,000 and
costs of investigation of $328.79, because he engaged in the practice of public accounting during a period

of $1,000 and costs of investigation of $396.56, because he engaged in the practice or offered to engage in
the practice of architecture in the Commonwealth or used the title of architect or used any sign, card or
devise implying he was competent to engage in the practice of architecture in the Commonwealth when

PA Board of Barber Examiners Berks County Detailed Cuts Barber Shop, license no. BO433820, of Reading, Berks County, was ordered to cease and
desist employing unlicensed individuals and from aiding and abetting unlicensed practice activity in the
practice of barbering; it was ordered to pay a civil penalty in the amount of $1,000 and an assessment fee
of $35.77 towards costs of investigation; and a public reprimand was placed on its permanent disciplinary

record with the Board, because it employed an unlicensed person and aided and abetted the unlicensed

York County Da Chop Shop, license no. BO433556, of York, York County, was ordered to cease and desist employing
unlicensed individuals and it shall further cease and desist from aiding and abetting unlicensed practice
activity in the practice of barbering; it was suspended for a period of one (1) year to practice as a barber
shop in the Commonwealth of Pennsylvania, such suspension to be immediately stayed in favor of
probation, subject to the Board’s terms and conditions; it was ordered to pay a civil penalty in the amount
of $1,000 and an assessment fee towards costs of investigation for $148.11, because it failed to employ
licensed persons; and it aided and abetted the unlicensed practice of barbering activities by Vernon

PA Board of Certified Real Estate Appraisers
Cambria County Donald Roger Hostetler, certificate no. RL000202L, of Salix, Cambria County, was ordered to pay a civil
penalty of $3,000, and to immediately cease and desist from performing any activity requiring a
certificate to practice as a general real estate appraiser or broker/appraiser, because he performed an
appraisal required by the Financial Institutions Reform, Recovery, and Enforcement Act of 1989 to be
performed by a State-certified or State-licensed real estate appraiser without holding the appropriate

PA Board of Chiropractic Philadelphia County Ethel C. Harvey, license nos. DC006838L and AJ006838L, of Philadelphia, Philadelphia County, had a
public reprimand placed on her permanent disciplinary record with the Board and was ordered to pay a
civil penalty of $2000, because she practiced on a lapsed license for a period of thirteen months which
also constituted a violation of her Consent Agreement and Order at paragraph 5(b)(1). (05/17/18)

Berks County Tina M. Stufflet, license no. CO164313L, of Hamburg, Berks County, was suspended for at least three (3)
years, with such suspension immediately stayed in favor of no less than three (3) years of probation,

Bradford County A Nails Inc, license no. CY198049, of Towanda, Bradford County, was ordered to cease and desist from
possessing Methyl Methacrylate substances, using metal razor tools in the practice of nail technology and
providing waxing services without the corresponding license and pay a civil penalty of $3,600, because it
operated as a nail technology salon in a grossly incompetent and unethical manner through the possession
of Methyl Methacrylate substances, using metal razor tools, failing to keep unused towels in a closed

Bucks County Andraya M. Dicicco, license no. CO258344, of Morrisville, Bucks County, was indefinitely suspended
immediately stayed to probation, based on her pleading guilty to three felonies. (03/15/18)
Glenn A. Ricci, license no. CO176444L, of New Hope, Bucks County, was ordered to cease and desist
from practicing cosmetology on a license that is lapsed, expired or otherwise not current with the
Pennsylvania State Board of Cosmetology; and he was ordered to pay a civil penalty in the amount of
$500, because he violated the law by practicing cosmetology without a current and active license.

penalty of $3,050, because it operated as a cosmetology salon in a grossly incompetent and unethical
manner by possessing a bottle labeled as containing Methyl Methacrylate (MMA); operated as a
cosmetology salon in a grossly incompetent and unethical manner by employing individuals not licensed
by the State Board of Cosmetology, with each individual found representing a separate count; the salon
practiced as an esthetician salon without a current and active license; it violated the law that it failed to
possess one (1) labeled first aid kit containing a blood spill container; and failed to possess one (1) first

Centre County Jasmine F. Bowders, license no. CO289348, of Rebersburg, Centre County, was suspended for a period of
Forty-Eight months, such suspension shall be immediately stayed in favor of probation subject to the
Board's terms and conditions, because she has been convicted of a felony. (05/07/18)
Dorothea A. Bird, license no. CT015252L, of Bellefonte, Centre County, was suspended for a period of
two (2) years, such suspension shall be immediately stayed in favor of probation subject to the Board's

manner by having a metal credo pedicure tool at a work station in the licensed nail technology salon

penalty of $4,050; and to cease and desist from possessing and using Methyl Methacrylate substances in
the practice of cosmetology, because it operated as a nail technology salon in a grossly incompetent and
unethical manner through the possession and use of Methyl Methacrylate substances in the practice of
cosmetology and failed to maintain the physical requirements of the salon by not having a complete first-

Lehigh County Best Nails Salon, license no. CY104250L, of Allentown, Lehigh County, was ordered to cease and desist
in the use of metal razor tools and pay a civil penalty of $750, because it failed to have tools, instruments,
utensils and appliances that come into contact with a client sanitized immediately after each use and
maintained in a sanitary condition at all times and operated as a nail technology and an esthetician salon
in a grossly incompetent, dishonest, and unethical manner through the use of metal razor tools. (05/07/18)
Ashley L. Shabazz, license no. CO252832, of Schnecksville, Lehigh County, was suspended for one (1)
year, with such suspension immediately stayed in favor of no less than one (1) year of probation, because
she violated the law by pleading guilty or nolo contendere to, or having been convicted of, a felony under

from possessing Methyl Methacrylate substances in the practice of nail technology and pay a civil penalty
of $750, because it failed to have a wet sanitizer, failed to maintain a closed storage area for soiled linen,
failed to contain a covered waste container, failed to properly sanitize razors for hair, tweezer, combs,
hairbrushes and other tools, instruments, utensils and appliances that come into contact, and operated in a
grossly incompetent and unethical manner through the possession of Methyl Methacrylate substances.

Montgomery County Larry H. Snyder, license no. DS019663L, of Hatfield, Montgomery County, was ordered to pay a civil
penalty of one thousand dollars ($1,000); pay costs of investigation of five hundred dollars ($500);
reimburse patients R.G. and K.G. the sum of two thousand dollars ($2,000); and was placed on probation
for a period of twelve (12) months, because of information provided to the State Board of Dentistry

Montour County Willis Clifton Manges, license no. DS020269L, of Danville, Montour County, had his license to practice
as a dentist in the Commonwealth of Pennsylvania suspended for a period of thirty months, such
suspension was immediately stayed in favor of probation; was levied a civil penalty of seven thousand
five hundred dollars and an assessment for the costs of investigation of two thousand one hundred thirty-
three dollars; and was required to complete remedial education regarding CDC guidelines, because he
committed unprofessional conduct by failing to follow the infection control recommendations issued by
the Federal Centers for Disease Control in his dental practice or failed to ensure that personnel and
