POSITIONOVERVIEWTheposition is responsible for individual support ina safeandsecure residentialenvironment forpeoplewho have experienced trauma, havemultiple complex needs and/or a diagnosedmental health issue ordisability.Thepositionisresponsiblefordailysupport,crisisintervention,informationprovision,programdevelopment,structureddailyactivities,administeringofmedication,householddutiesalongwitharangeof personal development programs supporting people to assist with their connection back into thecommunity.Supportisprimarilydeliveredtoclientsintheirownhome.casenotes,reportwriting,etc.

• To assist people we support to acquire/secure appropriate accommodation and support in thecommunity,
• Toprovide intensivesupport toenable thepeoplewesupport tomaintain theiraccommodation,minimiseharmtothemselvesandothers,achievetheirstatedgoalsandaddresstheirpsychosocial,emotionalandphysicalneeds,
• To ensure the program works cooperatively with other service providers to improve access toappropriate,relevantandtimelyservices,
• Toprovidecontinuityofcareandsupport,• To provide an appropriate organisational environment including staff support, development,
trainingandsupervision.
PRACTICEPRINCIPLES• Theprogramsendorsethekeyworkermodel,wherethedevelopmentofatrustingrelationshipis
seenaspivotaltothechangeprocess,andcomplementarytotherolesofadvocacyandsupport.• Programsalsoendorsethe“Programmatic”or“Team”approachtodirectservicedelivery,wherea
high degree of collaboration amongst teammemberswill contribute to the assessment of clientneedandthedesign,reviewanddeliveryofpackagesofsupport.
• Theprogramsaimtobestmatchcommunityresourcestothepeoplewesupport’sneeds.• Serviceprovisionisflexibleandrecognisesthatindividualneedschangeovertime.• The programs have a commitment to enhance the long-termhealth andwellbeing of peoplewe
support and recognise that people may need support of varying intensity and duration. Theprogramaimstoberesponsivetothisneed.
• Theprogramrecognisesthatduetoconstantorrecurringcrisis,peopledevelopcopingstrategies,whichmay inhibit theattainmentofastable lifestyle. Ermhaseekstovalidatetheseexperiencesandencouragesmoresustainableandeffectivesolutionstoachieveindependentliving,throughtheimplementationofanIndividualProgramPlan(IPP)developedforandwitheachconsumer.

• UndertakeandmaintainadirectservicecaseloadasdeterminedbyProgramManagement.
• Provideintensivesupportto:- Promotethecapacityofthepeoplewesupporttomaximisetheircontrolovertheirenvironment.- Enablethepeoplewesupporttoliveoptimallywithinasupportivecommunity- Facilitatethedevelopmentofsupportiveandresponsivecommunities,and- Facilitateconstructivelinkswiththerangeofcommunityservicesandresources.
• Contribute to the delivery of services which facilitate the development and/or rehabilitation ofpeopleandtheirconnectionintothecommunity.
• Assist with practical living skills including meal preparation and planning, laundry and personalhygieneandadministermedication.
• Maintain an appropriate level of safety within their residence for both staff and the people wesupport.
• Assistwiththeplanningandimplementationofarangeofstructureddaily&weeklyprogramsandactivitiestoassistwithconnectingbackintothecommunity.
• Develop and maintain effective professional working relationships with people who may exhibitchallengingbehaviouraltraits,tohelpsupportandmaintainpositivechange.
• Act as an advocate andmediator with other relevant services and professionals in regard to theneedsofthepeoplewesupport.
• Adhere to both internal and external legislative requirements and protocols in relationship toreferral,assessmentanddirectcaretasks.
• Establish and maintain a strong working relationship with family, significant others, communitysupportoptionsappropriatetothepeoplethatwesupportneedsandparticipateoncommitteesandinworkgroupswhicharerelevanttotheworkoftheprogram.
PlanningandReporting
• Maintain case management recording systems including case planning documentation, case notes,reports and data collection in accordance with Ermha procedure, policies and data recordingrequirements.

3. AnunderstandingoftheTraumaInformedCareframeworkandexperienceinbroadcasemanagementpracticesworkingfromarecoveryfocusedandyoungpersoncentredapproach.
4. Clear communication skillsbothwrittenandverbal. Theability toprepare clear, thoughtfulwrittenreports,maintain file anddata requirements and the capacity to represent the programat both anagencyandcommunitylevel.
