AWARD:

Two one thousand dollar (1,000) Scholarship will be awarded to the top McClain students meeting or exceeding the eligibility criteria below. The selection will be determined by a written application, the required documentation, and an interview with the Foundation Selection Committee.

DATE OF AWARD:

The scholarship award will be presented in May 2016. The 2016 recipients will be formally announced and presented with the Scholarship at that time.

ELIGIBILITY CRITERIA:

()High School Senior!

()Average GPA of 2.5 or higher (Attached an official copy of high school transcript with grades from the last 3 years.)

()Record of outstanding Community Service. !!

00 Two letters of Recommendation (one from a community member or

organization Knowledgeable about you, and one from a faculty member or school adminstractor.)

APPLICATION PROCESS:

To apply for this scholarship, complete the Application, Attach all documentation

and mail to the address by April 15, 2016. No Applications will be accepted after

this date..

- A biographical statement about yourself and your future career goals.

A statement detailing why you think you are deserving of this scholarship.

- An official copy of your high school transcript.

- Documentation of Community Service.

Two letters of recommendation (One from your school and one from a

Community member or Organization.)
