• Consistency across the state, while allowing for individual school and community needs

• Focused data collection

• Data that identifies true needs, gaps, and barriers

• Data that actually drives services and programs and leads to outcomes

• NEEDS ASSESSMENT DATA SHEETS

2 parts to needs assessment data sheets:

• Standardized datao Standard for every center, specific to center type o Possible source is identified

• Other data unique to the communityo Additional relevant data collected by center staffo Examples of each includedo Data expected in each of the categories (school,

county/district, FRYSC)o Local supporting data that highlights a significant

need or gap

*Will be included with the rest of the CPP

Changes include:

• Implementation section (formerly I and R)

NEEDS ASSESSMENT DATA SUMMARY

Coordinators should consider all relevant data identified on the needs assessment data sheet(s) and list top needs that will be addressed within the appropriate component.

This eliminates the need to retype data that may appear in multiple action components

Use measurable words – more, increase, decrease, improve, etc.

A simple, quick way to maintain coordinator awareness of the protective factors, and how their programs and activities contribute to building them.

CHANGES TO THE I AND R SECTION OF THE ACTION COMPONENT

Now in 2 parts:

• Implementation Report on Action Component Form• Impact Report on SurveyMonkey

Rationale:• Top request by coordinators according to Regional Advisory

Council survey• Reduces time spent on paperwork• Current I and R format was confusing with some parts

redundant (outcomes vs. outputs, data in Infinite Campus)• Outcome data was not in a useable format, not easily

compiled for sharing with stakeholders• KDE no longer uses this format

IMPLEMENTATION SECTION

The purpose of this section is

• to document if the activity was implemented

• to help the coordinator and Advisory Council make decisions on the activity’s effectiveness

Report date: Sept. 30, 2017 and 2018

IMPACT REPORT

Rationale:• To document demonstrable outcomes in a useful format

• To share statewide outcome data with stakeholders

• Data can be filtered by region, component, type of impact, etc.

• Allows for focused outcome reporting

• Only those activities that show clear outcomes are reported

• Does not force a response for every activity

IMPACT REPORT SPECIFICS

• SurveyMonkey, compiled automatically, able to be filtered

• Expectation that coordinators will evaluate significant activities

• Every center must submit AT LEAST one activity that was measured for change

• At a later date, a committee may be organized to decide which pieces of reported data should be publicized in an official report

OTHER CPP FORMS

Only minor changes including:

• Checklist – added the data sheet and inventory

• Assurance pages, minor language change

• Center Operations Form, Description “of center site”
